CCHL professionals are contracted with all major PPOs as in-network providers.  CCHL physicians are additionally in-network with Medicare.

Insurance carriers that we accept for M.D. and R.D. services:

• United Healthcare/Pacificare

Certain professional services are considered out of network–but in these situations, CCHL will submit the claims for the visits and credit the reimbursement to the patient.

The following services are not covered by insurance:

• Food consultant/chef
• Fitness trainers
