CCHL’S program allows its clients to rotate visits with the providers that are appropriate for their unique situation.

 

Initially, most of our patients will undergo the INTAKE process by one of our physicians.

INTAKE:  1-3 visits over 1-2 weeks.  The physician determines your goals, performs a complete assessment (medical and psychosocial), and then sets treatment goals with you and explains the program that will be set up to meet your goals.  Often the conceptual foundation for the program will be laid during this process as well.

 

FIRST BLOCK:  For many of our families who seek services at CCHL to address weight and body image concerns, we suggest that you rotate through services with each of our providers.  For those with more tailored needs (e.g. lipid management or prenatal nutrition), we will suggest visits with one or two providers.  Regardless, initial visits are usually 1 hour long–and provide an opportunity for ongoing assessment and initial intervention.   The initial BLOCK, which exposes you to each professional on a rotating basis (1-2 visits/week) ends with a visit with your physician.  At that visit, progress and goals are assessed, and the next block of services is designed collaboratively based on your needs.  A typical FIRST BLOCK for a patient with weight concerns would look as follows:

  • Week 1:  1 hour with Registered Dietitian
  • Week 2:  1 hour with Therapist
  • Week 3:  1/2 hour with Registered Dietitian follow up visit
  • Week 4:  1.5 hours with Food Consultant (in-home)
  • Week 5:  1/2 hour with Therapist follow up visit
  • Week 6:  1/2 hour with Physician follow up visit
  • At any point during the first 6 weeks, we recommend a consultation and follow up visit with our Fitness Professionals, who will oversee your exercise and activity.

 

We understand that you may not want or need consultations with one or more of these professionals–and that is ok.  This process is a collaboration, and your program will be tailored to your individual situation.

 

Following the FIRST BLOCK, it generally becomes clear which providers will be needed for ongoing work.  Services may continue once a week at that point, or may start to spread out.  In general, CHANGING HABITS REQUIRES TIME, and we hope that the majority of our patients will be with us in some capacity for at least 1 year.  This may end up being as little as a physician follow up every 3 months–or as much as regular visits for most of that year.  It is not always easy to predict at the beginning of the collaboration how intensive the relationship will end up being (or not).
